MUI Reporting

A Major Unusual Incident (MUI) is the alleged, suspected, or actual occurrence of an incident when there is reason to believe that the event has occurred and the health or welfare of an individual with a developmental disability may be at risk. MUIs must be reported to the County Board in a timely manner so that the incident can be investigated, and preventative measures can be put in place to help ensure the health and safety of the individual involved.

If you need to report an MUI, please contact our 24-hour reporting service at 740.497.2887.  In addition, you may want to contact the SSA involved. You can find contact information for that person on our staff directory page. If you are a provider and have notified the County Board about an incident, you will need to submit an Incident Report in writing to the SSA by 3:00PM on the following business day. You can record your report using DODD's blank Incident Report form.
